Qualtrics is an online survey tool that is available for free to all UCSF faculty, staff and students. It can be used to collect and store protected patient and personal data (PHI and PII). The Qualtrics Research Suite allows you to build, distribute and analyze online surveys no matter how simple or complex. Collaborate in real time, export data in multiple formats (e.g., SPSS, Excel, HTML, PDF and more) and integrate with existing Customer Relationship Management solutions (e.g., SalesForce).
Directions
- Log into the UCSF Qualtrics website (requires a MyAccess login). The system will create an account for you.
- By logging into Qualtrics, you certify that you understand and agree to the UCSF Qualtrics terms of use.
- Before creating a survey, determine if it requires Committee on Human Research (CHR) approval.
